Key Responsibilities:

• Conduct thorough patient assessments and create personalized care plans.
• Safely administer medications, treatments, and therapies following provider orders and facility protocols.
• Continuously monitor patient status, analyze clinical data, and take prompt action to changes in condition.
• Accurately document all assessments, interventions, and outcomes in the Electronic Health Record (EHR).
• Inform and educate patients and families about diagnoses, medications, procedures, and post-discharge care.
• Work collaboratively with physicians, advanced practice providers, and other team members to ensure safe and efficient care.
• Adhere to infection control standards, safety policies, and regulatory requirements.
• Engage in quality improvement initiatives and contribute to enhancing clinical workflows.
• Utilize technology-driven tools provided through Incredible Health’s platform for improved communication, scheduling, and care coordination.
• Facilitate smooth transitions for patients between levels of care and care settings, including discharge planning.
