We are on the lookout for an exceptional Nurse Care Coordination Specialist to join our dynamic and patient-focused healthcare team. Your main responsibilities will include:
- Assessing patient needs and developing personalized care plans
- Coordinating multidisciplinary services to promote comprehensive care
- Facilitating safe transitions across inpatient, outpatient, and community settings
- Reviewing clinical information and monitoring progress towards care goals
- Advocating for patient needs in collaboration with healthcare providers
- Educating patients and families on treatment options, resources, and follow-up care

Requirements and Qualifications
- Active RN license
- BLS certification (American Heart)
